All of the forms on the website are editable .pdfs. This means that you can type your information into them, save on your computer, print, and then mail the form in to the required address. You can also print the blank form and fill it in by hand.
	The MOUSBC Hall of Fame Nomination Form must be completed and postmarked no later than 10/15/2010 to be considered for this year's Hall of Fame Inductions.
	The Local Association Bowler of the Year Award is given to one male and one female bowler as nominated by their own local associations. DEADLINE 10/15/2010.
	The Missouri State USBC Proprietor of the Year Award annually recognizes an outstanding bowling center proprietor for his or her enthusiastic support of local and state associations. Regular form, not editable on your computer. DEADLINE 10/15/2010.
	The Local Association Volunteer of the Year Award will be presented yearly by the Missouri State USBC Association to recognize an outstanding member of a local association for their dedicated service in promoting the game of bowling. Four awards will be presented, one for male, female, boy, and girl. Missouri State USBC board members are ineligible for this award.. DEADLINE 10/15/2010.
	Two Local Association Youth Bowler of the Year Awards will be presented yearly by the Missouri State USBC Association to recognize a boy and a girl as Bowler of the Year, meeting the following criteria: Bowling Performance, Community Service, and Sportsmanship. Youth Missouri State USBC board members are ineligible for this award.. DEADLINE 10/15/2010.
	This Local Association Youth Volunteer of the Year Award is presented yearly by the Missouri State USBC Association to recognize an outstanding youth member of a local association for their dedicated service in promoting the game of bowling. Two awards may be presented, if qualified, one each for male and female. Missouri State USBC board members are ineligible for this award. Applicant can only receive this award one time. The deadline to submit the forms below has passed for the current year. Small changes are made yearly, and periodically, the forms are changed completely to meet the evolving needs of our organization. However, the forms are provided in case you want to use them as a guide for gathering information for the next year's application period.
	To apply as a director for one of our state tournaments, the MSUSBC Tournament Director Application Form must be completed, postmarked, and mailed to the state office by 02/15/2010. You can view the Tournament Director job description here. DEADLINE HAS PASSED.
	If you're interested in serving Missouri bowlers, you can apply to be a part of the Missouri State USBC Board of Directors. This Application Form must be completed and sent to each member of the Nomiating Commitee by 02/02/2010. Thanks for your interest! DEADLINE HAS PASSED.
	The MSUSBC Youth Committee Application Form must be completed by potential applicants and postmarked on or before 02/02/2010. DEADLINE HAS PASSED.
	The 2009 MSUSBC Showcase Registration form can be downloaded here. This form is due on 05/15/2010. DEADLINE HAS PASSED.
	The annual report for Officers for Local Associations is due to the state by 08/01/2010. You can either print the form and mail it, or now you can complete it online here. DEADLINE HAS PASSED.
